Applying for our Family Medicine Residency

We aim to provide the best training possible for our residents and to provide compassionate, high-quality primary care services to our patients, with an emphasis on caring for a diverse and underserved population here in Phoenix.  We review applications for evidence of service to patients and community, commitment to family medicine as a specialty, academic performance, and diversity of experience.  We seek to match candidates who share these values.

We participate in the National Resident Matching Program (NRMP) and we accept eight PGY-1 residents per year.  All applications must be sent through the Electronic Residency Application Service (ERAS). Paper or email applications will not be accepted by the program.

Application Criteria

U.S. citizens, permanent residents, green card holders, or J-1 Visa holders are welcome to apply.  Applicants need to meet the following criteria to be considered for an interview:

  • Graduate from an LCME, AOA, or WHO-accredited medical school within the last two years.
  • Pass both USMLE Step 1 and Step 2 or COMLEX 1 and 2 with no more than one failed attempt.  Candidates who have not passed both steps by the ranking deadline will not be ranked.
  • Submit three letters of recommendation. At least one letter of recommendation must be from a family physician.

Information for international Medical Graduates (IMGs)

  • Creighton will sponsor J-1 visas.
  • A minimum of three months of U.S. clinical experience (not counting research) is required.
  • IMGs do not need to have an ECFMG certificate to apply, but they must have one by the time they start residency.

What to Expect on Your Interview Day

All interviews for 2024-25 will be held virtually.

Interviews are conducted in the morning and are finished by 12:00 noon Arizona time. Interviews will begin with a program overview with all candidates together, followed by individual interviews with faculty and residents. There will also be a virtual "happy hour" on the evening of the interview that is attended by applicants and residents only. We strongly encourage all applicants to attend in the evening so they can meet our residents, as resident input is an important part of our interview process. If an applicant is unable to attend the happy hour on the day of their interview, attendance on another evening can be arranged.

Fourth-year Medical Student Sub-Internships

We offer MS-4 Sub-Intern rotations to qualified students. The rotation generally consists of 3 weeks working on our hospital inpatient service and one week in our outpatient clinic. Students will be asked to care for patients as if they were a PGY-1 resident, and they will have the opportunity to participate in rounds, write orders, arrange consults, and be involved in all aspects of patient care.

For more information, visit https://app.cliniciannexus.com/orgs/6933/valleywise-health and look for Family Medicine Sub-I or elective rotations at Valleywise Health in Phoenix.

Valleywise uses Clinician Nexus to manage our MS4 clinical experiences. Your use of the platform will be free to schedule rotations with us. If you are accepted for rotation, you will be required to complete all onboarding 90 days in advance of the rotation month. An affiliation agreement must be in place prior to a rotation. Please check with your school to see if there is an existing agreement in place.
