Become a Child and Adolescent Psychiatry Fellow

The Creighton-Phoenix Child & Adolescent Psychiatry Fellowship participates in the Electronic Residency Application Service (ERAS), which is supported by the Association of American Medical Colleges (AAMC). Applications will only be accepted through ERAS.   

Requirements

  • Three (3) years of General Adult Psychiatry Training at an ACGME-accredited institution.
  • Three letters of reference, including one from the program director
  • General (Adult) Program Director’s Attestation Form for Child and Adolescent Psychiatry (CAP) Fellowship Eligibility (to be filled out by Program Director and included with their letter of reference)
  • Program Director verification of completed experiences
  • Eligibility for license to practice medicine in Arizona
  • Step 3 successfully completed
  • J-1 visas accepted
  • Deadline for applications is November 1
