Tell Someone: Report an Incident or Concern

The Tell Someone online forms allow all members of the Creighton Community to report harassment, discrimination, grievances or concerning behavior. Users must login with a NetID and Blue password to utilize these forms. The information users provide will be private, and Creighton staff members who receive this form have signed a non-disclosure agreement, committing them to maintain the highest level of privacy. 

These forms should NOT be used to report an emergency. To report an emergency:

At the Omaha Campus
In an emergency or to report suspicious activity, please call Public Safety at (402) 280-2911. 

At the Phoenix Campus:
In an emergency or to report suspicious activity, call 911.
To report a crime or non-emergency 602-262-6151.
To call Silent Witness, dial 480-948-6377 (480-WITNESS).

At the Central Nebraska Campus or throughout the United States:

Cura Personalis, a Latin phrase meaning, “care for the person” or “personal care,” is having concern and care for the personal development of the whole person and dedication to promoting human dignity. It describes the type of care we give as educators and healthcare professionals.

This Help Someone form is rooted in Cura Personalis. It exists to share concerns about students attending Creighton’s Phoenix Health Sciences Campus. This form is also available to help students in need of additional support.

This form may be used to request support or express concern for others or for yourself. To submit a request for support, report a concern, or report misconduct, please use this online reporting form.

Report Behavioral Misconduct on the Omaha Campus
Behavioral misconduct violations include, but are not limited to, alcohol, drugs, hazing, vandalism, and other violations of university policies. To report behavioral misconduct, please use this online reporting form.

Report an Incident of Title IX Sexual Harassment
Title IX sexual harassment includes incidents of sexual assault, dating violence, domestic violence, and stalking.  The Office of Title IX and Civil Rights Compliance receives reports works to address incidents and connect individuals to resources and reporting options. To report an incident, please use this online reporting form.

Report an Incident of Bias or Discrimination
The Bias Education and Support Team (BEST) is comprised of members from the Office of Title IX and Civil Rights Compliance, Community Standards and Student Conduct, the Division of Equity, Diversity, and Inclusion, and Department of Human Resources. BEST reviews reports of bias-related incidents or behavior, and addresses incidents of bias within the Creighton community. Report a Concern about a Student
Concerns for students may include mental health, self-harm, substance misuse, food insecurity, financial hardship, threatening behaviors, and much more. Reports are reviewed by Community Standards and Student Conduct in consultation with the Office of Title IX and Civil Rights Compliance, Student Counseling Services, and other relevant campus partners. Faculty, staff, and students are encouraged to report concerns about students using this online reporting form.

Report a Concern about a Staff or Faculty Member
Concerns for staff or faculty may include mental health, substance misuse, concerning workplace behaviors, and more. Reports are reviewed by the Department of Human Resources in consultation with the Office of Title IX and Civil Rights Compliance, Department of Public Safety, and other relevant campus partners. Faculty, staff, and students are encouraged to report concerns about a staff or faculty member using this online reporting form.

Department of Residential Life Report Form
This form is for use by Residential Life staff, including Resident Assistants and Resident Directors. If you are not a member of the Residential Life team, please review the report types above and submit your concern there.

Student Leadership and Involvement Center Report Form
This form is for use by SLIC staff and student leaders, including fraternity and sorority presidents, executive board members, etc. If you are not seeking to report a concern related to a fraternity, sorority, club, or student organization, please review the report types above and submit your concern there.

Campus Security Authority Report Form
