The Path to Support Begins Here

Community Connections is committed to building one-on-one relationships with young people and families. We offer free, voluntary support by providing information about resources and connecting you to other community organizations that can assist you. We believe that all families deserve access to resources—and that they know what’s best for themselves.

We’ll Follow Your Lead

When you partner with us, you lead the relationship. Our Community Connectors are here to help you determine strengths, needs and capacity for participation. They also remove barriers and identify both informal and formal supports. Community Connectors are reflective of the communities where families live, serving as vital links to the supports for youth and families.

Create community-based partnerships that are family focused, culturally sensitive and empower individuals and families to improve their quality of life and promote the restoration of a strong sense of community.

Support Community Connections

Community Connections updates its Amazon Wish List regularly with a variety of items needed for youth and families. Purchased items can be sent directly to our offices for quick inventory and dispersion (select the Community Connections address when choosing a shipping address).
