Tell Someone allows all members of the Creighton community to use simple online forms to report harassment, discrimination, grievances or concerning behavior. Any information provided in these forms will be private; Creighton staff members who receive Tell Someone forms have signed a non-disclosure agreement—committing them to maintain the highest level of privacy.
These forms should NOT be used in an emergency. At the Omaha campus, you can report an emergency or suspicious activity, by calling Public Safety at 402.280.2911. At all other locations, please dial 911.
Tell Someone forms are tailored to different types of concerns or campus locations. You will find links to forms for specific concerns in the section below. You can access these forms with your NetID and Blue password. Currently, there is one form for the Phoenix campus. All other forms are for the Omaha campus.
Students at Creighton’s Health Sciences campus in Phoenix can use this form to request support for themselves, or report misconduct and concern for other students.
Use this form to report violations that include, but are not limited to, alcohol, drugs, hazing, vandalism, and other violations of university policies.
This form allows you to report sexual assault, dating or domestic violence, or stalking. The Office of Title IX and Civil Rights Compliance receives these reports and works to address incidents. The office can also connect individuals to resources and additional reporting options.
With this form, you can report incidents and behavior related to bias within the Creighton community. Reports are reviewed by the Bias Education and Support Team (BEST), which is comprised of members from the Office of Title IX and Civil Rights Compliance, Community Standards and Student Conduct, the Division of Equity, Diversity, and Inclusion, and Department of Human Resources.
Creighton faculty, staff, and students can use this form to report concerns related to a students’ mental health, self-harm, substance misuse, food insecurity, financial hardship, threatening behaviors, or other concerns. Reports are reviewed by Community Standards and Student Conduct in consultation with the Office of Title IX and Civil Rights Compliance.
You can use this form to report a concern about a faculty or staff member’s mental health, substance misuse, workplace behaviors, and more. Reports are reviewed by the Department of Human Resources in consultation with the Office of Title IX and Civil Rights Compliance, Department of Public Safety, and other relevant campus partners.
