Uploading Additional Information

The Human Resources team at Creighton wants to ensure the information we gather is kept safe and secure. Depending on your circumstances, our team may ask you to provide us with private and personal information. To do this, the University uses JayDrop to send and receive large files quickly and securely.

If you prefer, any completed form can be dropped off or sent to the Human Resources office located in the Linn Building at 511 N 20th Street in Omaha, or through interoffice mail. You may also fax forms to us at 402.280.5516.

For your protection, any document containing birth dates, Social Security numbers or account information can no longer be accepted via unsecured email.
