Tips for New Bluejays

Students are required to upload immunization records to the BirdHouse health portal by May 15th to ensure eligibility for course registration. Please review Creighton University’s immunization requirements.

When ready to upload, login to the BirdHouse portal using your netid@creighton.edu email address. Once logged in, enter your immunization dates AND upload the supporting documentation from the doctor’s office. No specific form is required. Please direct all questions to immunizations@creighton.edu. 

Nebraska is one of two states where 19 is the age of majority, instead of 18. This medical power of attorney form, to be signed and notarized by a parent or guardian, allows students who are 18 and younger to give consent for their own health care.

Those who choose to complete this form should do the following: 

1. Email the completed form to studenthealth@creighton.edu. (A copy of the form will be kept on file in Student Health Education and Compliance and a copy will be sent to the CHI Student Care Clinic.) 
2. Students should carry a copy of this form or keep a digital image of this form on their phone, should they seek healthcare in Nebraska, outside of the CHI health system.

Microsoft Teams is the main method of instant message communication your small group leader will use to communicate during your session. Download the app on your phone or desktop or access it through the Creighton Office 365 suite. Your Net ID and Blue password will give you access to this app.
